The home DNA testing kit includes a Peace of Mind DNA test request/consent/payment form, complete instructions on how to collect your DNA samples and send them to the lab, a DNA sample collection package containing two swabs, and an addressed ready-to-mail envelope to get your DNA test samples directly to the laboratory with the 9 laboratory processing fee.
Instructions
1. Take one of the swabs and firmly rub it onto the inside of your left cheek for a minimum of 20 seconds, then place it into the swab envelope. Repeat using the second swab on the inside of your right cheek and add it to the same envelope.
2. Seal the swab envelope which should have the relevant details already completed on the front.
3. When all samples are collected, put the swab envelopes and completed form in the return envelope and mail back to AlphaBiolabs
along with the 9 laboratory processing fee.
